Program Overview

Case Management is a free, flexible, and personalised support service for young people ages 12 to 25 living in the Perth metropolitan area.

Through one-to-one support, case workers help young people navigate challenges, build confidence, and develop the skills they need to flourish.

Support is tailored to each young person’s unique needs and can include assistance with housing, employment, education, health, wellbeing, and accessing services such as Centrelink.

WHAT TO EXPECT

The Y WA Case Management program follows a simple and supportive step-by-step process, so you always know what to expect.

1. Referral and Allocation 

Complete a case management referral form to get started. You’ll then be matched with a case worker who’s the right fit for your needs.

2. Assessment (1-4 sessions) 

You’ll meet with your case worker to talk about the areas where you want support such as employment, housing, education, health, or connecting with services like Centrelink.

During this phase, your case worker will:

  • Learn about your strengths and goals
  • Understand any risks or safety concerns
  • Complete a safety plan with you if needed

This helps build a clear picture of how we can best support you.

3. Case Planning (1–2 sessions) 

Together, you’ll set SMART goals with clear strategies and timelines. Your case plan is flexible and built around your needs, interests, and priorities.

4. Monitoring and Review

You’ll have regular check-ins depending on your situation to review your progress, adjust plans as needed, and stay on track to meeting your goal.

5. Case Closure 

When you’ve reached your goals, your case will close. You’ll:

  • Reflect on your achievements
  • Discuss your next steps
  • Receive tools and resources to help you move forward confidently

If you need support again in the future, you can re-engage with the service.

WHERE IS IT AVAILABLE

  • You can access case management support at the following Y WA locations:
  • We also provide case management support in local high schools across the City of Armadale, City of Belmont, City of Vincent, and Shire of Serpentine Jarrahdale
  • If you’re not enrolled at a school in one of these areas, we can arrange to meet you in a safe public place, or you’re welcome to come by our office

  • There are no pre-requisites to get started with the Y WA Case Management

HOW TO GET STARTED

  • To begin, simply complete the case management referral form
  • Once your referral is received, a case worker will be assigned to you
  • Together, you’ll arrange a convenient and safe place to meet

With the guidance of a trained case worker, young people can get help with areas such as housing, employment, education, health, wellbeing, and navigating services like Centrelink.

Case management is a completely free service. 

We accept referrals from: 

  • Young people 

  • Parents and caregivers 

  • Schools 

  • Government agencies 

  • Health professionals 

  • Community and social service providers 

All referrals are confidential and will not be shared without the young person’s consent. 

All Y WA case workers hold at least a Certificate IV in Youth Work or a related field, with many completing qualifications such as a Diploma or Bachelor’s degree in social work, youth work, psychology, counselling, or community services. 

They also complete training in: 

  • Mental Health First Aid 

  • Suicide Intervention 

  • Trauma-informed Practice 

  • Culturally safe practice  

  • Youth Worker Code of Ethics 

  • First Aid 

All staff hold a Working with Children Check and Police Clearance. 

Yes. Parental or caregiver consent is required for young people under 18 accessing case management services. 

Most young people meet with their case worker weekly. However, the frequency is flexible: 

  • Increased sessions for young people needing urgent support.

  • Reduced sessions (fortnightly or monthly) as goals are achieved or support needs decrease. 

There is no fixed time limit. Young people can receive support for as long as their goals and circumstances require. Some may only need short-term assistance, while others benefit from ongoing support due to more complex needs. 

You can access case management support at the following Y WA locations:  

  • Base@Belmont Youth Centre (for ages 12–25) – 275 Abernethy Rd, Cloverdale.

  • The Y HQ Leederville (for ages 12–25) – 60a Frame Ct, Leederville. 

  • Y Time Armadale (for ages 12–18) – Unit 5, 36–40 Commerce Ave, Armadale.

We also provide case management support in local high schools across the City of Armadale, City of Belmont, City of Vincent, and Shire of Serpentine Jarrahdale.  

 This includes schools such as Mt Lawley Senior High School, Churchlands Senior High School, Bob Hawke College, Kelmscott Senior High School, Cecil Andrews College, Armadale SHS, Byford Secondary College, and Belmont City College. 

When delivered in schools, case management is provided on-site in a private, safe location. Sessions are scheduled in consultation with the student, school, and case worker, and occur as often as needed based on the young person’s goals and level of support required. 

The Y WA has been delivering case management support for over 10 years.  

Our case management support is flexible and tailored to each young person’s needs.

Support may include: 

  • Goal Setting & Achievement: Identifying personal goals and taking practical steps to reach them. 

  • Education & Learning Support: Assistance with school, re-engagement, alternative pathways, and skill development. 

  • Employment Support: Job readiness, resume help, work experience, and finding employment opportunities. 

  • Accommodation & Housing Support: Guidance on safe housing options and referrals. 

  • Mental Health & Wellbeing: Strategies for coping, self-care, and links to professional services. 

  • Alcohol & Other Drugs Support: Connections to relevant services and harm-minimisation strategies. 

  • Sexual Health Information: Confidential support and access to reliable information. 

  • LGBTQIA+ Support: A safe, inclusive space with referrals to specialist services. 

  • Safety Planning & Risk Assessment: Personalised safety planning if a young person feels unsafe. 

  • Centrelink & Financial Support: Help understanding payments, applications, and entitlements. 

  • Advocacy & Referrals: Speaking up for young people and connecting them with the right services. 

  • Early Intervention & Prevention: Addressing challenges early to build resilience and avoid crisis situations.
